HOW TO BOOK WITH YOUR LIVING WELL PROVIDER

WELCOME BACK TO LIVING WELL INTEGRATIVE HEALTH CENTRE!

PLEASE READ AS WE ARE OPENING IN ALIGNMENT WITH STRICT PUBLIC HEALTH REGULATIONS 

 

We know that the past three months have been a challenging time for our community. We have missed you and we are so happy to be able to reopen Living Well to be able to serve your health and wellness needs.

 

We are committed to providing the best care possible for you and your family. But, due to the continuing dangers of spreading Covid-19, we are taking precautions to safeguard everyone’s health.

  • NO WALK-INS & NO EMERGENCY VISITS: We are not accepting any WALK-IN patients until further notice. If you are having an EMERGENCY and must see a doctor, please call 811, 911 or go to the emergency room at the QEII or IWK.

 

  • APPOINTMENT ONLY: All patients must make an appointment in advance to see Dr. Patriquin,  or Ian Shepard. Make an apt by logging onto: https://healthmyself.ca/ If you do not have access or an account or are a new patient please email lwihc.halifax@gmail.com with your specific information and inquiry and you will receive a call within 2 days. 

 

  • ADDENDUM JUNE 10, 2020: Please note that you are responsible for keeping your personal contact information (phone, address & email) up to date. If you cannot access an apt because of this please contact the office by emailing your detailed information. For any difficulty booking with health myself there is a contact help center number on the site. Kindly contact them and they will walk you through the process. If you had a referral sent in the past 1-3 years and your contact information has changed it is your responsibility to contact the respective specialists offices to notify them as in the event that they have mailed your apt notification to an outdated address, you will have effectively missed your apt and need to rebook or end up back on a waiting list. 

 

  • The phone is accessed remotely twice a week and is reserved for those without any internet access or other barriers to accessing care. 

 

  • TELE-HEALTH: Your appointment with Dr. Patriquin MD (family medicine, mindfulness, stress reduction, counselling, psychotherapy, trauma informed care), Ian Shepard (physiotherapy & acupuncture), Leila Shaw (registered dietician), Lisa Galvez (registered dietician), Robin Stamm (breath work & yoga) or Molly De Shong (counselling & mindfulness) will be a tele-health visit. You will be emailed a link once you have established with your provider which platform you want to use.

 

  • IN-PERSON APPOINTMENTS IF NEEDED: If Dr. Patriquin or Ian Shepard decide that you need to been seen in person, staff will make a follow-up appointment for you to come into the Living Well office to be seen.

 

  • HEALTH AND SAFETY RULES: Before your appointment, you will receive the Office Visit Health and Safety Rules for an in-person visit.  The Rules are mandatory. You must follow the Rules in order to come into the Living Well office – your safety and the health of the wider community depends on everyone respecting the Rules.

 

  • ALL HEALTH SERVICES AVAILABLE: Dr. Patriquin is able to provide all of her usual medical and counseling services by phone – prescription renewals, referrals to specialists, examining patients with illness or chronic health needs, and counselling. MSI covers all of her tele-health appointments.

 

IF YOU HAVE SYMPTOMS OF Covid-19 OR BEEN IN CONTACT WITH SOMEONE WHO HAS PLEASE CALL 811.

DON'T PUT OFF BEING SCREENED.
