Collaborative Care

 

COLLABORATION CARE is about us working together to identify and address your personal and health related needs and goals.

 

WHAT WE DO:

We work hard to coordinate the resources and care you feel you need from the various providers at Living Well as well as with other providers you see in the community.

We share knowledge only with your consent. We communicate only if it is understood that sharing knowledge ensures we are able to provide you with the best possible care. 

 

WHAT WE DON’T DO:

We don’t share information you specifically ask a provider not to share regardless of signed consent to communicate.

We don’t talk to each other every time you see one of us. If you know you have an appointment and want to make sure we are up to date with your health status, please let your provider know in advance that you want communication before your next appointment. If it is possible, we will try and do this. 

We don’t take away your autonomy. Collaborating does not mean we choose and decide. It is your choice who you see, what concerns you address, and ultimately what you do with the advice that is provided. It's all about YOU! 

 

YOU HAVE THE MOST IMPORTANT ROLE IN COLLABORATING:

The most important collaboration is the one you form with each of your providers. This means first and foremost it is up to you to take an active role in communicating your needs, concerns, and goals.
